Normal Birth Registration  #

Reason of Application #

Peninsular Malaysia

  • The registration of birth must be made within 60 days of the birth of a child in Malaysia.

Sabah

  • The registration of birth refers to registration that is made within 14 days of the birth of a child in Malaysia.

Sarawak

  • The registration of birth refers to registration that is made within 14 days of the birth of a child in Malaysia

Application Condition #

Application for registration of a child may be submitted by the following persons:

  • Parents
  • Legal Guardian
  • Person having knowledge of the birth
  • The mother and father of the child (for the registration of births illegitimate child.

Procedure #

The applicant may head over to any National Registration Department counter with the required documents.

Peninsular Malaysia #

  • Child’s Documents (Original and Photocopies)
    • Birth Registration Form JPN.LM01 that is completed.
    • Confirmation of birth form from the hospital or certification of home birth from the midwife / doctor (original)
    • Prenatal card (maternity examination book) (original and photocopy)
  • Parent’s Documents (Originals and Photocopies)
    • Identity cards or entry permits or passports of the parents and the person reporting the birth (Originals and Photocopies)
    • Death Certificate of mother and father of the child (Original and Photocopy if applicable)
    • Marriage or divorce certificate (Originals and Photocopies)

Sabah

  • Child’s Documents (Original and Photocopies)
    • Form A and N1 (extract) that are completed

Late Birth Registration – Peninsular Malaysia #

Reason of Application #

Late Registration of Births made more than 60 days after the date of birth of child in Peninsular.

Application Condition #

Application for registration of a child may be submitted by the following persons:

  • Parents
  • Mother or father and a sponsor with knowledge of the birth who is 15 years older than the child (if one of the parents cannot be found / is deceased)

Procedure #

Application for late registration of birth can be made throughout Peninsular Malaysia by bringing along the necessary documents to any NRD office within the applicant’s place of residence.

Childs’ Documents (Originals and Copies)

  • Birth Registration Search Form JPN.LM12 (for births from 2002 onwards)
  • Birth Registration Form JPN.LM01 that is completed
  • Confirmation of birth form [Born in Government / Private Hospital or Clinic)
  • Prenatal card or maternity examination book
  • Child’s Heath and Immunisation Card
  • Certification of home birth by midwife / doctor (originals and photocopies)
  • 1 recent full length 4R photograph
  • 1 recent passport sized photograph
  • Certification of birth from a headman or village chief or Member of Parliament or member of a State Legislative Assembly for the constituency where the child was born.
  • Extract of school register or school certificate with endorsement from the head teacher or school principal if the child is schooling and aged 7 and above, or alternatively, certification from a headman or village chief or Member of Parliament or member of a State Legislative Assembly for the constituency where the child resides, that the child has never been schooled and is aged 7 and above.
  • Statutory Declarations of 2 witnesses with knowledge of the birth. [Home Birth]

Late Registration of Births made more than 42 days after the date of birth of child in Sabah.

Application Condition #

Application for registration of a child may be submitted by the following persons:

  • Parents
  • If the father is not available, then the mother or a sponsor with knowledge of the birth who is 10 years older than the child
  • If the father is not available and the mother has died, the application may be submitted by 2 sponsors with knowledge of the child, who are both aged 18 and above and are at least 10 years older than the child.
