Application For Citizenship Under Article 14 Of The Federal Constitution Confirmation Of Citizenship (Born in Malaysia) #
This is an application to confirm the status of a person born in Malaysia on or after Malaysia Day (16 September 1963) based on the status of the mother or the father.
REQUIREMENTS #
- The applicant and the person to be registered shall appear before the Registrar of Citizenship/consular officer when submitting the application in accordance with Rule 28 of the Citizenship Rules 1964 (Amendment 1996).
- The applicant shall be the:
- parent;
- guardian or next of kin; or
- the person to be registered
- Form E must be completed and submitted together with copies of the necessary documents.
PROCEDURE #
The applicant and the person to be registered are required to go to the counter at the Citizenship Division of the Putrajaya NRD headquarters or state NRD headquarters or NRD branch, bringing along originals and copies of required documents.

- Birth Certificate or latest extract of Birth Certificate
- Identity card (if any)
- Passport or other travel document (if any)
- Documentary evidence of schooling (if applicable)
- Three (3) recent passport-sized photographs with a blue background
- Parents’ Documents (Originals And Copies)
- Birth Certificate or latest extract of Birth Certificate (if any)
- Identity card (if any)
- Entry permit or passport or other travel document (if any)
- Citizenship Certificate (if any)
- Marriage document (Muslim couples who married in a foreign country must submit confirmation by the Islamic Religious Department)
- Sibling’s Documents (Originals And Copies)
- Birth Certificate or latest extract of Birth Certificate
- Identity card (if any)
Note: All copies of supporting documents should be made on A4 paper.

- The applications for citizenship matters require the use of original forms of Form E. Forms may be obtained from the NRD in Putrajaya or State NRD headquarters/branches where the registration is made in Malaysia, and the Malaysian Mission overseas where the registration is made outside of Malaysia.
